The patient presented with a skeletal and dental Class III malocclusion due to maxillary retrusion, associated with anterior and posterior crossbite, generalized spacing, and relatively small tooth dimensions. During presurgical orthodontic preparation, the dentition was aligned and coordinated while two planned spaces were maintained in the lower anterior region to compensate for the reduced tooth size and preserve adequate arch dimensions and functional tongue space, supporting long-term stability. This was followed by 6 mm maxillary advancement, resulting in correction of the occlusion and significant improvement of the facial profile.
